Medium filler is a term used by some manufacturers to describe short filler without the stem bits. Often these are trimmings from premium tobacco leftover from the manufacturing process. Private Stock Medium Filler is a cigar from Tabadom that uses trimmings from much more expensive cigars made in the same factory. According to them anyway. Mixed filler is a combo of long and short filler and short is, well, just short fill so they are nowhere near the same.
