Those are a couple things I've personally had problems with that caused me to have to relight constantly. The draw of the pipe should be like "sipping soda from a straw".

Not to mention packing a bowl properly takes a bit of practice. It took me awhile before I was able to pack a bowl and have it stay lit for more than a puff or two.
