Still looking for the "best", but a few from memory that were always good.
DiGennaro's - Reno, NV (sold to a family that switched over to a mexican restaurant....then closed.)

Blondie's in Berkeley.

Up here in the PNW, there are a few places that do O.K.(The Rock, Pagliacci, NEO,) but still haven't found one that I'd rate higher than a 6 on a 1-10 scale.
